Determination of Ability to Pay Our staff will assist you with scheduling an appointment to determine your ability to pay. For this appointment you will need your Medicare and/or Medicaid card, other insurance cards if you have other insurance benefits, and proof-of-income documents. Proof of income could be recent paycheck stubs or your latest income tax returns. Your Obligations You are legally obligated to pay charges for services received from SCCMHA based upon the amount determined to be your ability to pay. You are also obligated to provide the insurance company explanation of benefits statement and pay SCCMHA for any payments you receive from your insurance company. Services are billed monthly and payment is expected within 30 days of billing. We are willing to work out an alternative payment schedule if circumstances justify a needed delay in payment.
