Certified Community Behavior Health Clinic (CCBHC) Services

Who is eligible for CCBHC services?

CCBHC services are available to any person in need, including but not limited to individuals with serious mental illness, serious emotional disturbance, long-term chronic addiction, mild or moderate mental illness, and substance use disorders. Individuals meeting these criteria are eligible for CCBHC services regardless of residency or ability to pay.

What is a Certified Community Behavioral Health Clinic (CCBHC)?

A CCBHC is an organization or entity that aims to provide increased access to high-quality behavioral health care and physical health care to individuals with mental health and substance use disorders. The services provided by a CCBHC utilize evidence-based practices to focus on wellness, recovery, trauma-informed care, and physical-behavioral health integration.

What services are provided by a CCBHC?

As a CCBHC site, SCCMHA is responsible for ensuring each of the following services are made available, either by providing them directly, or through contractual relationships with Designated Collaborating Organizations (DCOs). 

  • Crisis Services (including 24-hour mobile crisis teams, emergency crisis intervention services, and crisis stabilization)
  • Patient-Centered Treatment Planning (including risk assessment and crisis planning)
  • Screening, Assessment, Diagnosis & Risk Assessment
  • Outpatient Mental Health & Substance Use Services
  • Targeted Case Management
  • Outpatient Primary Care Screening and Monitoring of Key Health Indicators and Health Risk
  • Community-Based Mental Health Care for Veterans and Members of the Armed Forces
  • Peer, Family Support & Counselor Services
  • Psychiatric Rehabilitation Services
