 *MHFA Registration Form* 

• To register for a class, you must complete and follow the direction on the Registration Form for your preferred class date. 
  • Virtual/ Blended Courses: registration and payment is required at least 10 days prior to the scheduled virtual training date in order to be processed in a timely manner. 
  • All other courses: registration and payment are required 48 hours prior to the scheduled start date of the course. 
• Those outside of Saginaw County, Michigan...$30 payment is required before confirming a course registration is sent. 
• All yellow sections of the registration form must be completed in order to be considered successfully registered. You will receive a confirmation email once your registration is processed. 
• Please remember that all course times are Eastern Time Zone.
