| **This form is used to notify SCCMHA Continuing Education to Add a new staff training record for your provider. This form must be submitted before your staff begins SCCMHA Online training. Providers will also use this form to Remove terminated staff from your provider training records. This will also remove the staff from your required training reports. |

The "training" button will allow you to review the training before taking the quiz. The "quiz" button will allow you to complete the quiz. Once you complete the quiz you will receive the notice of completion and score. That score is automatically delivered to a training inbox for data entry - no further action is required of you.
