  **This form is used to notify SCCMHA Continuing Education to Add a new staff
training record for your provider. 
This form must be submitted before your staff
begins SCCMHA Online training. 
Providers will also use this form to Remove
terminated staff from your provider training records.  This will also remove the
staff from your required training reports.
